Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs and physician direction. They are responsible for the delegation and direction of all discipline-specific therapy services for the assigned patients, ensuring functional treatment programs are designed for effective returns to community activities. This role also involves serving as an educator for patients, families, students, coworkers, and self, and performing other professional duties associated with patient care and therapy service development.

Responsibilities

• Complete comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans and goals.
• Collaborate with interdisciplinary teams to address barriers to discharge.
• Manage individual, concurrent, and group treatment sessions effectively and efficiently.
• Document patient care in a clear, concise, accurate, and timely manner.
• Participate in a student program, providing a positive learning environment.
• Actively pursue professional growth through education and professional activities.
• Adhere to discipline-specific State Practice Acts and maintain licensure.
• Engage in patient and professional advocacy activities.
• Understand and implement organizational performance improvement initiatives.

Required Experience / Certifications / Licensure

• BLS Certification required prior to the first shift.
• Must hold a valid, current State license as a Physical Therapist.
• Prior experience in acute care and inpatient rehabilitation settings preferred.
• Proficiency in EPIC (Electronic Medical Records) is required.

Schedule

This position follows a Day shift schedule of 5x8 hours.
