Job Description

The Travel Physical Therapist will provide essential therapy services to patients in a skilled nursing facility. This position requires a dedicated professional to deliver outstanding patient care, perform evaluations, and develop treatment plans for rehabilitation.

Responsibilities

• Conduct patient evaluations and develop individualized treatment plans
• Assist patients with activities of daily living (ADLs) and transfers
• Provide education to patients and families regarding treatment plans
• Implement gait training and therapeutic exercises
• Ensure compliance with documentation standards using Meditech (Expanse)

Required Experience / Certifications / Licensure

• Must hold a valid Physical Therapist (PT) license
• Current Basic Life Support (BLS) certification required
• Experience in a skilled nursing facility (SNF) preferred
• Ability to effectively function in a fast-paced environment
• Strong adaptability and independence in clinical responsibilities
