Physical Therapist

Job Description

The Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs and physician directions. This position is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that promote an effective return to community activities. The therapist serves as an educator to patients, families, students, and coworkers while performing other professional duties associated with patient care and therapy service development.

Responsibilities

• Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
• Collaborate with the interdisciplinary team to address all barriers to discharge, including timely equipment trialing and ordering.
• Conduct patient and family training, including the issuance of home exercise programs, and plan for continuing services.
• Effectively manage individual, concurrent, and group treatment sessions based on patient care needs.
• Document medical records accurately and timely, including daily documentation and discharge paperwork.
• Participate in student program activities by providing a positive learning environment and appropriate supervision for students.
• Actively engage in professional development through continuing education and self-study and apply learned information to meet organizational objectives.
• Adhere to discipline-specific State Practice Act and maintain professional licensure in good standing.
• Understand and contribute to organizational performance improvement initiatives aimed at enhancing quality of services.

Required Experience / Certifications / Licensure

• BLS Certification Required Prior to First Shift
• Must hold a valid, current State license
• Minimum of 2 years of experience required
• Previous LTACH experience preferred

Schedule

This position operates on a Day 5x8.0 schedule and is expected to start the day after Labor Day.
