Monday through Friday, 08:00 - 17:00 (Day 5x8-Hour). No weekends or holidays.

Job Description

We are seeking a dedicated Registered Nurse for a Home Health position to provide quality care to patients in Montgomery County. The ideal candidate will possess experience in HCHB (Pointcare) and OASIS paperwork.

Responsibilities

• Provide nursing care to patients in a home health setting.
• Conduct patient assessments and evaluations.
• Complete necessary documentation and OASIS paperwork accurately.
• Collaborate with interdisciplinary teams, including social services and interpretation services.
• Follow a schedule to manage daily patient visits effectively.

Required Experience / Certifications / Licensure

• 2 years of nursing experience preferred.
• Must have HCHB (Pointcare) experience.
• Active Maryland or Compact RN license required.
• Tobacco-free facility; candidates must be nicotine-free.
• Two supervisor references required upon submission.
