5x8-Hour Shift (08:00 - 17:00), Monday to Friday

Job Description

We are seeking a dedicated Physical Therapist for a home health position. The ideal candidate will have experience in home health settings and be able to manage adult health care cases efficiently. You will be expected to complete 6 visits per day, providing essential therapy services in the comfort of patients’ homes.

Responsibilities

• Conduct evaluations and develop treatment plans for adult patients.
• Implement therapy interventions in home settings.
• Document patients' progress and adjust care plans as needed.
• Collaborate with interdisciplinary teams, including social services.

Required Experience / Certifications / Licensure

• Current Maryland Physical Therapist state license required.
• BLS certification required.
• Minimum of 1 year of home health experience preferred; first-timers accepted.
• Experience with HCHB charting system is a plus.
