Job Description

The Case Management Coordinator is responsible for managing routine case management, discharge planning, swing bed coordination, and utilization review duties. This role involves evaluation and screening of potential admissions to the facilities' swing bed program and requires knowledge of criteria for Medicare, Medicaid, HMO, swing bed, and private insurance coverage.

Responsibilities

• Initiate ongoing communication with residents and their families to assess discharge needs.
• Actively participate in daily rounds with bedside staff, physicians, and ancillary team members.
• Communicate with physicians to ascertain plans for timely discharge and document updates as part of ongoing planning.
• Manage and collaborate with the healthcare team on swing bed placements and complete MDS documentation as necessary.
• Assist in obtaining referrals and prior authorizations for Home Health Care, CME, SNF, acute rehab, and appointments.
• Educate physicians and staff on appropriate levels of care/utilization and required documentation for admission status.
• Run Interqual for each inpatient and observation patient admitted to the floor.

Required Experience / Certifications / Licensure

Experience in case management and knowledge of insurance coverage criteria is required. Relevant certifications may be preferred.
