Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patient's needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective return to community activities. As an educator, the therapist engages with patients, families, students, coworkers, and themselves, while performing other professional duties associated with patient care and program delivery.

Responsibilities

• Complete a comprehensive evaluation of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans focused on progressing patients toward discharge.
• Document medical records clearly, concisely, and accurately, including goal writing, quality assessments, and charge entry.
• Provide care under signed, valid physician orders and ensure compliance with insurance requirements.
• Participate in student programs by offering a positive learning environment and appropriate supervision.
• Create an individualized professional development plan and pursue continuing education and professional growth.
• Adhere to state practice acts and maintain professional licensure in good standing.
• Understand and apply the organization's performance improvement methodology to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active license to practice physical therapy in the relevant state.

Schedule

This is a 5x8.0 hours shift position.
