Schedule

8:00 AM - 5:00 PM, with on-call requirements every 6-7 weeks from 5 PM to 8 AM during that week and the weekend.

Job Description

The Home Health Registered Nurse will be responsible for providing high-quality nursing care to patients in their homes. The position requires home health experience and hospice experience, particularly for nursing roles. The candidate must have a reliable vehicle for patient visits.

Responsibilities

• Deliver direct nursing care to patients in home settings.
• Conduct assessments and develop care plans tailored to patient needs.
• Administer medications and treatments as prescribed.
• Participate in patient and family education regarding medical conditions and care.
• Maintain accurate and up-to-date patient documentation using HCHB.

Required Experience / Certifications / Licensure

• Must have home health and hospice experience for nursing positions.
• Registered Nurse (RN) License required.
• Experience in home health or hospice care is essential.
