Schedule

Monday to Friday, 8:00 AM - 5:00 PM, with call requirements every 6-7 weeks for one week at night (5 PM - 8 AM) and that weekend.

Job Description

We are seeking dedicated individuals for nursing and therapy positions in home health and hospice care. We do not make night visits from this office, and reliable transportation is necessary for patient visits.

Responsibilities

• Provide comprehensive care to home health and hospice patients.
• Conduct assessments, develop treatment plans, and ensure proper medication administration.
• Engage in therapies and interventions for rehabilitation and health maintenance.
• Maintain accurate documentation in the HCHB documentation system.

Required Experience / Certifications / Licensure

• Home health and hospice experience required for nursing positions.
• Home health experience required for therapy positions.
• Valid RN or therapy licensure as appropriate.
• Experience with patient care and case management is preferred.
