Job Description

We are seeking a dedicated Home Health Registered Nurse (RN) to provide compassionate care and support to patients in their homes. The ideal candidate will possess strong assessment skills and experience in home health settings, particularly with cardiopulmonary, geriatrics, neuro, ortho, and oncology patients.

Responsibilities

• Conduct 6-8 home visits per day, delivering skilled nursing care and patient education.
• Perform comprehensive assessments and develop individualized care plans.
• Administer medications and manage patient family education.
• Document patient care and progress using BayCare Forms and Ace Timesheet system.
• Address safety and injury prevention strategies during home visits.
• Participate in interdisciplinary collaboration to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Minimum 2 years of home health experience required.
• OASIS training preferred.
• Valid state RN license required; compact license accepted.
• BLS certification is mandatory.
• Must possess a valid driver's license and have reliable transportation for home visits.
• COVID-19 vaccination required; exemptions considered with appropriate documentation.
• Flu vaccination or declination required with mandatory masking policy in place.
