Outpatient Physical Therapist

Schedule

This position offers a 5x8.0 schedule, with Monday through Friday shifts totaling 40 hours per week.

Assignment Duration

This is a 12-week contract position, starting on August 1st.

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on patient needs, goals, and physician direction. The therapist will be responsible for delegating and directing all discipline-specific therapy services. This role emphasizes the design of functional treatment programs, facilitating effective community reintegration. The therapist also serves as an educator for patients, families, students, coworkers, and themselves, and is expected to adhere to the organization's mission and core values, including strict compliance with safety standards.

Responsibilities

• Conduct comprehensive evaluations of assigned patients.
• Utilize clinical reasoning to develop appropriate treatment plans focused on reaching discharge goals.
• Document all medical records accurately and in a timely manner, including evaluations, treatment, and quality indicators.
• Participate in the student program, providing supervision and evaluation as needed.
• Create a professional development plan in collaboration with clinical management.
• Adhere to the discipline-specific State Practice Act and maintain licensure in good standing.
• Demonstrate knowledge of performance improvement methodologies to enhance service quality.

Required Experience / Certifications / Licensure

• A degree in Physical Therapy from an accredited institution.
• At least 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure for Physical Therapy.
