This is a 13-week contract position.

Schedule

Variable shifts: 8:30 AM to 4:30 PM or four 10-hour shifts from 7:30 AM to 5:00 PM. No call required but weekend and holiday rotation is needed.

Job Description

We are seeking experienced RN Care Managers to coordinate utilization management activities and serve as the primary link between patients, healthcare teams, and payers during hospital stays. Ideal candidates will demonstrate a commitment to improving patient outcomes through effective care management practices.

Responsibilities

• Conduct comprehensive assessments of patient needs and secure necessary resources for transitioning patients.
• Collaborate with an interdisciplinary team in daily care rounds and planning conferences.
• Communicate effectively using written, verbal, and electronic systems.
• Work with community organizations to arrange durable medical equipment, insurance authorizations, and transportation.
• Participate in managing the length of stay for the inpatient population.
• Understand hospital reimbursement methods and quality measures relevant to care management.

Required Experience / Certifications / Licensure

• Associate or Bachelor degree in Nursing.
• 3-5 years of clinical experience in medical/surgical nursing, home care, or hospital case management.
• Experience in utilization review and discharge planning is required.
• Certification by a nationally recognized case management or utilization review organization is preferred.
• Active Basic Life Support (BLS) certification is mandatory.
• Case Management Certification (CCM) or Accreditation in Care Management (ACM) is preferred.
