Day Shift: 5x8-Hour (08:00 - 17:00), Monday through Friday

Job Description

A Physical Therapist is needed for home health services. Candidates must have home health experience, and experience with HCHB software is preferred. The role involves managing a caseload of adult patients in their homes.

Responsibilities

• Conduct evaluations and develop treatment plans for adult patients.
• Provide therapy interventions in home settings.
• Maintain documentation and report progress within required systems.

Required Experience / Certifications / Licensure

• Valid Maryland Physical Therapist State license required.
• BLS certification required.
• Minimum of 1 year of home health experience required.
• Two supervisor references required.
