Outpatient Physical Therapist

Job Description

The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. Responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, the therapist ensures the design of functional treatment programs allowing for effective returns to community activities. Responsibilities include serving as an educator for patients, families, students, and coworkers while maintaining a commitment to patient and employee safety.

Responsibilities

  • Complete comprehensive evaluations of assigned patients and utilize clinical reasoning to develop appropriate treatment plans and goals.
  • Document medical records in a clear, concise, accurate, and timely manner following state, federal, and organizational guidelines.
  • Ensure care is provided under signed and valid physician orders within applicable insurance approvals.
  • Participate in student programs, providing a positive learning environment and supervision as assigned.
  • Create an individualized professional development plan and actively pursue professional growth opportunities.
  • Adhere to the discipline-specific State Practice Act and other regulatory requirements, maintaining licensure in good standing.
  • Engage in performance improvement initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

  • Degree in Physical Therapy from an accredited school.
  • Minimum of 1 year of outpatient clinical experience.
  • Current CPR certification.
  • Active relevant state licensure to practice as a Physical Therapist.

Schedule

This is a 5x8 hour shift position starting on 9/15.
