Day shift, Monday to Friday, with 8-hour days from 08:00 to 16:30.

Job Description

We are seeking a dedicated Home Health Registered Nurse to join our team in York County. The ideal candidate will have a strong background in home health care, with the ability to provide quality nursing services to patients in their homes. The role requires excellent assessment and critical thinking skills, along with the ability to document patient care effectively.

Responsibilities

• Conduct patient assessments and evaluations in a home health setting.
• Develop and implement individualized care plans based on patient needs.
• Monitor and document patient progress and response to treatment.
• Utilize OASIS documentation as required.
• Provide education and support to patients and their families regarding care and treatment options.

Required Experience / Certifications / Licensure

• Minimum of 1 year of nursing experience, preferably in home health.
• OASIS documentation knowledge preferred but not required.
• Active RN license in the state of Pennsylvania.
• COVID-19 vaccination is required; medical and religious exemptions are allowed.
• Flu vaccination required; medical and religious exemptions only.
