Licensed Practical Nurse (LPN) - Full Time Days and Nights

Job Description

The Licensed Practical Nurse (LPN) provides direct and indirect skilled nursing care for assigned residents, following defined policies and procedures under the supervision of a Registered Nurse. The ideal candidate will demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served, particularly focusing on adult and geriatric populations.

Responsibilities

• Collect and communicate data relevant to the resident's physical and psychosocial condition to the RN and interdisciplinary team.
• Contribute to the development of a plan of care that is age-appropriate and culturally relevant, reflecting nursing diagnoses and interventions.
• Recommend revisions to care plans based on evaluations of resident outcomes.
• Collaborate with residents and families to ensure satisfaction and comprehensive care addressing all aspects of the resident's needs.
• Maintain current knowledge and competency in nursing practice through continuing education and in-service programs.
• Provide leadership and mentoring to staff under the direction of the RN, considering safety, costs, and impact in planning care delivery.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure
• Current BLS certification (AHA or RQI)
• Minimum of 1 year experience as an LPN, preferably in a long-term care facility

Schedule

Full-time position with a schedule of 5 days, including days and nights, with every other weekend and holiday required.
