Day shift, 5x8-hour schedule (08:00 - 16:30), required to work every other weekend.

Job Description

We are seeking an experienced Case Management Registered Nurse to join our team. Ideal candidates will have a strong background in acute care and possess the necessary skills to provide high-quality patient management and coordination. The role requires floating between like units as needed based on departmental requirements.

Responsibilities

• Conduct assessments and develop comprehensive care plans for patients.
• Coordinate services across multiple disciplines to ensure continuity of care.
• Manage patient admissions, discharges, and transitions effectively.
• Ensure compliance with regulatory requirements and standards of care.
• Educate patients and families about care processes and available resources.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in acute care nursing.
• Current RN license.
• Experience in case management, including familiarity with admission criteria, discharge planning, and care coordination.
• Knowledge of CMS regulations and familiarity with Epic charting system is required.
