Acute Care Case Manager
Job Description
This is a 13-week contract position. We are seeking an experienced Acute Care Case Manager to provide care coordination and discharge planning in an acute care setting. The ideal candidate will possess a strong background in hospital case management with a focus on optimizing patient care transitions.
Responsibilities
- Perform comprehensive care coordination and discharge planning for patients in an acute care facility.
- Utilize CMS criteria and InterQual standards for patient assessment and planning.
- Organize and facilitate family meetings and involve stakeholders in the care plan development.
- Manage referrals to community resources, including DME, SNF, and transportation services.
- Assess patients’ needs, including those facing mental health challenges, substance use issues, or homelessness.
Schedule
This position follows a Day 5x8-Hour schedule from 08:00 to 16:30 with weekend coverage required to meet department needs.
Required Experience / Certifications / Licensure
- 2 years of acute care hospital case management experience is required.
- Current California state license is mandatory, pending licenses will be accepted if obtained by the start date.
- Experience with Epic charting system is preferred.
- Knowledge of drug and alcohol treatment options and advance directives is beneficial.
- Must commit to working 3 of the 5 holidays (Thanksgiving, Christmas Eve, Christmas Day, New Year’s Eve, New Year’s Day).
