Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans based on the patients' needs, goals, and physician directions. This role requires delegation and direction of all discipline-specific therapy services for those assigned patients, ensuring a design of functional treatment programs that facilitate an effective return to community activities. The therapist will serve as an educator for patients, families, students, coworkers, and themselves, while also performing professional duties related to patient care and therapy services development.
Responsibilities
- Complete comprehensive evaluations and develop individualized treatment plans and goals.
- Document patient care in a clear, concise, accurate, and timely manner.
- Provide care under signed, valid physician orders and within insurance approvals.
- Participate in student programs and provide a positive learning environment.
- Actively pursue professional development through continuing education and involvement in professional activities.
- Adhere to the discipline-specific State Practice Act and maintain professional licensure in good standing.
- Understand and apply performance improvement methodologies to enhance care quality.
Schedule
This is a full-time position with a 5x8.0 schedule, working 40 hours per week.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 6 months to 1 year of outpatient clinical experience.
- Current CPR certification.
- Active and applicable state license.
