Day shift; 5x8 hours (08:00 - 17:00).

Job Description

We are seeking a compassionate and skilled Home Health Registered Nurse (RN) to provide exceptional care to our patients in a home setting. The ideal candidate will be responsible for executing personalized patient care plans, supporting patients' health needs, and ensuring the highest quality of care.

Responsibilities

• Conduct thorough assessments of patients' physical, emotional, and psychological needs.
• Develop and implement comprehensive care plans in collaboration with the healthcare team.
• Administer medications and monitor patient progress.
• Educate patients and families about health management and safety precautions.
• Utilize various medical equipment and devices necessary for patient care.
• Document patient interactions and care provided accurately.
• Perform skilled home care visits, ensuring necessary documentation and billing accuracy.

Required Experience

• Must have at least 1 year of home health experience.
• OASIS training preferred.
• Experience with various patient conditions including med/surg, cardiac, orthopedic, and respiratory.

Certifications & Licensure

• Active RN license in the state or a compact license.
• Current Basic Life Support (BLS) certification.
