Registered Nurse Case Manager

Schedule

7:30 AM - 4:00 PM, with every third weekend from 7:30 AM - 4:00 PM.

Job Description

We are in need of a Registered Nurse Case Manager to begin on May 11th. The focus will be on Care Coordination and Discharge Planning, in addition to Utilization Review. This position is critical to managing length of stay and overall patient efficiency within the department.

Responsibilities

• Coordinate patient care and ensure smooth discharge planning processes.
• Conduct utilization reviews to maximize resource efficiency.
• Manage patient cases in cooperation with interdisciplinary teams.
• Support acute clinical operations and ensure adherence to care protocols.

Required Experience / Certifications / Licensure

• Must have at least 4-5 years of experience as a Registered Nurse.
• A minimum of 3 years of acute clinical experience is required, in addition to case management experience.
• Previous travel assignments as a case manager (2-3 preferred).
