Licensed Practical Nurse (LPN)

Job Description

The Licensed Practical Nurse (LPN) will provide direct and indirect skilled nursing care for assigned residents, following established policies and procedures under the supervision of a Registered Nurse (RN). The role requires the ability to demonstrate knowledge of principles of growth and development across the lifespan, particularly for adult and geriatric patient groups.

Responsibilities

• Collect data relevant to resident's physical and psychosocial condition and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of culturally relevant care plans and involve resident/family in this process.
• Recommend revisions to care plans based on evaluations of resident outcomes, ensuring comprehensive management of care needs.
• Maintain knowledge and competency in nursing practice through continuing education and participation in mentoring/orientation of staff.
• Demonstrate leadership in the professional nursing setting and ensure a safe, customer-focused environment.

Schedule

• Candidate must be available to start on a Tuesday or Thursday, working full shifts.
• Minimum of 24 hours per 4 weeks; 2 weekend shifts required.

Required Experience / Certifications / Licensure

• Current NY LPN Licensure.
• BLS certification.
• Minimum of 1 year of experience as an LPN at a Long-Term Care Facility.
