Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans according to the patient's needs, goals, and physician direction. This role involves delegating and directing all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that facilitate effective returns to community activities. The therapist serves as an educator for patients, families, students, coworkers, and themselves while adhering to professional standards and codes of conduct.

Responsibilities

• Complete comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals.
• Document patient care clearly, concisely, accurately, and timely, ensuring compliance with relevant guidelines and policies.
• Participate in student programs as appropriate, providing supervision and evaluations.
• Develop an individualized professional development plan and pursue ongoing education and professional involvement.
• Maintain professional licensure and adhere to discipline-specific State Practice Act and regulatory agency standards.
• Support performance improvement initiatives and provide feedback to enhance service quality.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• 1+ year of outpatient clinical experience.
• Current CPR certification.
• Active licensure for the state where work will be performed.
• Previous experience with this agency is highly desired.
