Registered Nurse - Home Health/Hospice

This is a 13-week contract position.

Schedule

Day Shift: 5x8-Hour (08:00 - 17:00) with an on-call rotation during weekends.

Job Description

We are seeking a compassionate and skilled Registered Nurse to provide exceptional care in a home health and hospice setting. The ideal candidate will be responsible for managing care for patients with terminal illnesses, focusing on their quality of life and end-of-life care needs.

Responsibilities

• Conduct comprehensive intake assessments and perform medication administration.
• Manage symptoms effectively through pain management techniques and interventions.
• Provide goal setting and education to patients and their families.
• Conduct symptom assessments and manage various patient diagnoses.
• Actively participate in a multidisciplinary team to ensure high-quality patient care.

Required Experience / Certifications / Licensure

• Minimum of 2 years of experience in Home Health, Hospice, or Skilled Nursing settings.
• Current BLS certification.
• Active RN license in the applicable state, or Compact license.
• Experience with medication administration and symptom management.
• Familiarity with EPIC charting system preferred.
