Case Manager (Registered Nurse) - Day Shift

Job Description

This is a 13-week contract position for a Case Manager, serving in a registered nursing capacity. The role focuses on discharge planning and case management within an acute hospital setting. The selected candidate will provide comprehensive assessments, planning, implementation, and overall evaluation of individual patient needs, enhancing the quality of patient management and satisfaction while promoting continuity of care and cost-effectiveness.

Responsibilities

• Conduct comprehensive assessments and develop individualized discharge plans.
• Coordinate care and resources for patients to optimize outcomes and satisfaction.
• Ensure compliance with regulatory and departmental guidelines.
• Perform concurrent and retrospective reviews to validate medical necessity.
• Utilize EPIC for documentation and care coordination purposes.

Schedule

Day shift, 5x8 hour schedule (Monday to Friday). No call requirements and minimum weekend coverage.

Required Experience / Certifications / Licensure

• Minimum of 4 years of experience as a Registered Nurse, with at least 2 years in a healthcare setting.
• Registered Nurse license to practice in North Carolina.
• BLS certification required; ACM/CCM preferred.
• EPIC experience required.
• No first-time travelers; candidates must reside outside a 50-mile radius from the facility.
