Job Description

The Patient Care Support Specialist plays a key role in enhancing the patient experience by collaborating with the healthcare team and ensuring efficient support for patient needs. This position demands strong communication skills, the ability to manage multiple tasks, and a focus on building trust with patients and their families.

Responsibilities

• Work collaboratively with the Registered Nurse Case Manager (RNCM).
• Utilize effective documentation skills in various systems including CERNER, NATE, and ENSOCARE.
• Demonstrate strong communication skills to engage with patients, families, and the healthcare team.
• Shift attention between multiple tasks as needed to ensure seamless patient care.
• Build trust and rapport with patients and their families to enhance the care experience.

Required Experience / Certifications / Licensure

No specific certifications are required. However, relevant experience in a healthcare setting and proficiency in communication and documentation are essential.
