Job Description

As a Physical Therapist, you will be responsible for providing superior care while adhering to safety policies and procedures.

Responsibilities

• Conduct comprehensive evaluations of assigned patients and develop appropriate treatment plans and goals focused on discharge within an established length of stay.
• Collaborate with an interdisciplinary team to address discharge barriers, including equipment trials, patient and family training, and planning for continuing services.
• Manage individual, concurrent, and group treatment sessions based on patient care needs effectively and efficiently.
• Document medical records clearly, accurately, and timely, including daily notes, goals, and discharge documentation in compliance with guidelines.
• Participate in student programs, providing mentorship and supervision while fostering a positive learning environment.
• Engage in professional development by creating an individualized growth plan and pursuing continuing education opportunities.
• Attend to patient and professional advocacy efforts including community health events and professional memberships.
• Maintain professional licensure and meet state-mandated continuing education requirements.
• Understand and apply performance improvement methodologies and participate in initiatives to enhance the quality of services provided.

Required Experience / Certifications / Licensure

• BLS Certification required prior to first shift.
• Must hold a valid, current State license to practice as a Physical Therapist.
• A minimum of 2 years of experience is required.
• Previous inpatient rehab experience preferred.
