Job Description

We are seeking an experienced Registered Nurse with a background in Skilled Nursing Facilities (SNF) who is familiar with NetHealth (Rehab Optima). The ideal candidate will have a strong commitment to providing quality patient care and must hold a currently active California license.

Responsibilities

• Deliver comprehensive nursing care to patients in a skilled nursing facility setting.
• Conduct patient assessments and develop individualized care plans.
• Administer medications and perform medical procedures as necessary.
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes.
• Maintain accurate patient records and documentation in compliance with regulations.

Required Experience / Certifications / Licensure

• Active California Registered Nurse (RN) license.
• Previous experience in a Skilled Nursing Facility (SNF) preferred.
• Familiarity with NetHealth (Rehab Optima) is preferred.
