Day Shift: 8-hour days (08:00 - 16:30). Note: This facility cannot accommodate 4x10s or 3x12s.

Job Description

We are seeking a dedicated Home Health Physical Therapist to provide exceptional care to patients in Medford, OR, and surrounding areas. The ideal candidate will be open to new travelers, including those with no prior home health experience.

Responsibilities

• Deliver one-on-one patient care with a standard productivity point system.
• Travel to patients' homes while managing a maximum radius of 50+ miles occasionally.
• Document patient care using the EPIC/Oasis charting system.

Required Experience / Certifications / Licensure

• BCLS certification required.
• Open to travelers with no home health experience; first-timers accepted.
• Oasis experience preferred; please indicate if not available.
• Candidate must have reliable transportation.
• COVID-19 vaccination primary series and one booster required.
