Job Description

We are seeking a dedicated and skilled Physical Therapist (PT) for a home health position. The role involves providing physical therapy services to patients in their homes, ensuring optimal patient outcomes through evaluation and treatment planning.

Responsibilities

• Conduct initial evaluations and develop individualized treatment plans.
• Implement therapy interventions consistent with the plan of care.
• Document all patient interactions and treatment progress in compliance with standards.
• Participate in case conferences and interdisciplinary collaboration.
• Ensure compliance with OASIS documentation requirements.

Required Experience / Certifications / Licensure

• Home Health and OASIS experience required.
• Experience with Homecare Homebase documentation preferred.
• Current licensure as a Physical Therapist.
