Outpatient Care Provider

Schedule

07:30 AM - 05:00 PM, primarily weekdays with occasional Saturday coverage (typically 2 Saturdays per 13-week contract).

Job Description

We are seeking a qualified Outpatient Care Provider to deliver exceptional patient care while ensuring that the highest standards are maintained. The ideal candidate will have experience in outpatient settings and possess the required licensure to practice in Oregon.

Responsibilities

• Provide direct patient care within the outpatient setting.
• Administer medications and other therapeutic interventions as needed.
• Document patient interactions thoroughly and accurately.
• Collaborate with healthcare team members to develop and implement patient care plans.
• Conduct assessments and evaluations to ensure optimal patient outcomes.

Required Experience / Certifications / Licensure

• BLS Certification.
• Active Oregon State license as required for the role.
• Completion of required health screenings and background checks prior to onboarding.
• Successful passing of Trinity Specialty Specific Competency Testing.
