Outpatient Physical Therapist
Job Description
The Outpatient Physical Therapist evaluates all assigned patients and develops, implements, and modifies treatment plans in accordance with the patients' needs, goals, and physician direction. This role is responsible for the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring the design of functional treatment programs that allow for effective return to community activities. The therapist serves as an educator for patients, families, students, coworkers, and self, adhering to organizational Mission Statement and Core Values.
Responsibilities
- Completes a comprehensive evaluation of assigned patients.
- Utilizes clinical reasoning to develop appropriate treatment plans and goals.
- Maintains clear, concise, accurate, and timely medical record documentation.
- Ensures care is provided under signed, valid physician orders and within insurance approval.
- Participates in the student program as assigned, providing a positive learning environment.
- Actively pursues professional development through continuing education and self-study.
- Adheres to the discipline-specific State Practice Act and maintains professional licensure.
- Demonstrates awareness of organizational performance improvement initiatives.
Required Experience / Certifications / Licensure
- Degree in Physical Therapy from an accredited school.
- 1+ year of outpatient clinical experience required.
- Current CPR certification.
- Active and applicable state licensure.
Schedule
This is a 5x8.0 position with shifts from 8:00 AM, offering a structured work schedule.
