Registered Nurse - Hospice Care

Assignment Duration

This is a 13-week contract position.

Schedule

Tuesdays to Saturdays, preferably from 10 AM to 7 PM or 11 AM to 8 PM.

Job Description

The Registered Nurse will provide hospice care and perform patient visits as needed. In the absence of admissions, the RN will focus on completing scheduled visits within the coverage area of San Bernardino County, potentially extending to other territories within a 60-mile radius as required.

Responsibilities

• Conduct patient visits and assessments for hospice care.
• Maintain productivity expectations of 24-30 visits per week for a 40-hour work week.
• Provide high-quality care while ensuring compliance with organizational policies.
• Document patient interactions and care provided using HomeCare HomeBase software.

Required Experience / Certifications / Licensure

• Must have valid RN license.
• Experience in hospice care is required.
• HomeCare HomeBase experience is preferred; alternative arrangements may be considered for applicants without this experience.
• Valid driver's license and auto insurance are required.
