Occupational Therapist -Home Health

Job Description

CVHCare is seeking a qualified Occupational Therapist to join our home health team. This position is ideal for new graduates as well as those with home health experience. The Occupational Therapist will provide therapy services in the assigned county territory, which includes Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, and additional areas in Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont, San Ramon, Dublin, Pleasanton, Sunol, and Livermore.

Responsibilities

• Conduct evaluations, develop treatment plans, and therapy interventions for patients in home health settings.
• Complete OASIS assessments and ensure compliance with healthcare regulations.
• Maintain productivity by completing at least 27 points per week through scheduled visits and documentation.
• Manage a full caseload and be willing to travel outside the area or to work weekends as needed to meet visit expectations.
• Work with a diverse patient population, including Covid+ and Covid PUI patients.

Required Experience / Certifications / Licensure

• Current California Occupational Therapist License
• BLS Certification
• Valid Driver's License and Auto Insurance
• Annual TB test and immunizations (MMR, Varicella, HepB, TDap, Flu)
• Drug screen and physical examination as required

Schedule

This position operates primarily during daytime hours, with the potential for weekend coverage required based on patient demand.
