Job Description

CVHCare is seeking a qualified Physical Therapist to provide outstanding care in various locations, including but not limited to Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City, and Oakland. Additional travel outside these areas may be required.

Responsibilities

• Conduct evaluations, create treatment plans, and provide interventions as required.
• Ensure compliance with documentation and patient care standards.
• Manage patient caseload effectively, which may involve occasional travel outside the primary service area.
• Be willing to treat COVID-positive and COVID-patient under investigation cases.

Required Experience / Certifications / Licensure

• Current California Physical Therapist License.
• Basic Life Support (BLS) certification.
• Valid Driver's License and proof of auto insurance.
• Current TB screening (annual), MMR, Varicella, HepB, TDap (10 years), and annual Flu vaccination.
• Completion of a drug screening and physical may be required.
