Schedule

Day shift: 8-hour days from 08:00 am to 04:30 pm. Weekend work is not required. Please note that this facility cannot accommodate alternative schedules such as 4x10s or 3x12s.

Job Description

We are looking for a skilled Home Health Occupational Therapist to join our team. The ideal candidate will provide quality care to patients in the comfort of their homes, focusing on improving their physical activities and daily living skills. The position requires a 1:1 patient ratio and may involve visiting up to 6 patients per day.

Responsibilities

• Deliver individualized therapy to clients in home health settings.
• Assess patients’ needs and develop appropriate treatment plans.
• Utilize evidence-based practices and standardized assessments.
• Engage in patient and caregiver education regarding treatments and home exercises.
• Document patient progress and modify treatment plans as necessary using EPIC/Oasis.

Required Experience / Certifications / Licensure

• A minimum of 1 year of skilled home health experience is required.
• BCLS certification is mandatory.
• Oasis experience is required; please specify when and where this was obtained.
• Applicants must have reliable transportation as this role involves travel between patient homes.
• Primary series COVID vaccination and one booster are required.
