Job Description

We are seeking a skilled Physical Therapist Assistant with home health experience to deliver quality care to patients in their homes. The candidate will work with a diverse patient population, including those with medical, surgical, orthopedic, neurological, and respiratory conditions.

Responsibilities

• Provide physical therapy services to patients in home settings.
• Conduct assessments and create treatment plans.
• Utilize a variety of patient care equipment and devices.
• Educate patients and families on home exercise programs and safety.
• Document patient progress and treatments as per guidelines.
• Collaborate with interdisciplinary teams to ensure holistic care delivery.

Required Experience / Certifications / Licensure

• Minimum of 1 year experience in home health.
• Valid Florida state license or compact license.
• Basic Life Support (BLS) certification required.
• OASIS training preferred.
• Must be COVID vaccinated or provide an exemption form.
• Flu shot required; declination must be accompanied by mandatory mask-wearing at all times while at work.
