Day shift, Monday through Friday, from 08:00 or 08:30 to 16:00 or 16:30.

Job Description

We are seeking a dedicated Case Manager RN to provide and coordinate medical home care, placement, and social services for safe discharges. The ideal candidate will have a strong background in inpatient hospital or acute care settings, with a focus on a wrap-around service model for patient care management.

Responsibilities

• Manage and coordinate care for an average of 15 cases daily.
• Assess patient needs and develop comprehensive care plans.
• Facilitate safe discharge planning and placement services.
• Collaborate with interdisciplinary teams to address medical and social issues.
• Utilize EPIC charting for patient documentation.

Required Experience / Certifications / Licensure

• 3 years of related experience required; will also consider first-time travelers with relevant inpatient experience.
• Active RN license in Maryland or compact license required.
• BLS certification preferred.
