Job Description

The Case Manager RN is responsible for providing and coordinating medical home care, placement, and social services for inpatient case management to ensure safe discharges. The role requires 3 years of experience in an inpatient hospital or acute care setting, although first-time travelers with relevant experience will be considered.

Responsibilities

• Coordinate patient care and ensure safe discharge planning.
• Provide case management services for patients with complex medical and social needs.
• Utilize EPIC charting for patient records and documentation.
• Address medical home care, placement, and social issues as part of wrap-around services.

Required Experience / Certifications / Licensure

• Active Maryland RN license required.
• Basic Life Support (BLS) certification preferred.
• 3 years of relevant inpatient case management experience.
• Experience with EPIC charting is preferred.
