Job Description

The Outpatient Physical Therapist evaluates assigned patients and develops, implements, and modifies treatment plans based on the patients' needs, goals, and physician direction. This role includes the delegation and direction of all discipline-specific therapy services for assigned patients, ensuring functional treatment programs are designed for effective community reintegration.

Responsibilities

• Conduct comprehensive evaluations and develop treatment plans focused on patient progress towards discharge.
• Document patient care accurately and timely, ensuring compliance with state, federal, and organizational guidelines.
• Ensure care is provided under valid physician orders and within insurance approvals.
• Participate in student programs, supervising and mentoring students effectively.
• Create and pursue individualized professional development plans.
• Adhere to professional standards and maintain licensure requirements.
• Foster a safety-oriented environment, prioritizing patient and employee safety.

Required Experience / Certifications / Licensure

• Degree in Physical Therapy from an accredited school.
• Minimum of 1 year of outpatient clinical experience.
• Current CPR certification.
• Active and applicable state licensure as a Physical Therapist.
• Previous experience with the agency is highly desired.
