Full-Time Licensed Practical Nurse (LPN) - Night Shift

Full-Time Nights: 5x8 per week, including every other weekend and holidays.

Job Description

The Licensed Practical Nurse (LPN) will provide comprehensive nursing care to adult and geriatric patients. The candidate must possess the knowledge and skills necessary to assess and interpret data related to the patients' physical and psychosocial status, ensuring tailored care to meet age-specific needs.

Responsibilities

• Collect data relevant to residents' conditions and communicate findings to the Registered Nurse and interdisciplinary team.
• Participate in the development of culturally relevant care plans that reflect nursing diagnoses and intervention strategies.
• Evaluate resident outcomes and suggest necessary revisions to care plans, ensuring client satisfaction through collaboration with residents and families.
• Maintain current competency in nursing practices through engagement in continuing education and in-service programs.
• Participate in the mentoring and orientation of new staff, demonstrating leadership within the nursing practice setting.

Required Experience / Certifications / Licensure

• NY LPN Licensure
• BLS certification (AHA or RQI only)
• A minimum of 1 year of experience as an LPN, preferably in a long-term care setting
