Full Time variable shifts including day and evening hours, 5 days a week, with weekend and holiday rotations required.

Job Description

The Licensed Practical Nurse (LPN) is responsible for providing direct and indirect skilled nursing care to assigned residents, ensuring that care is appropriate to the age-specific needs of adult and geriatric patients. The LPN works under the supervision of a Registered Nurse and maintains a collaborative relationship with other healthcare professionals.

Responsibilities

• Collect and assess data relevant to residents' physical and psychosocial conditions.
• Communicate findings effectively to the Registered Nurse and interdisciplinary team.
• Participate in the development of culturally relevant care plans in collaboration with the healthcare team and residents/families.
• Evaluate resident outcomes and recommend revisions to care plans as necessary.
• Mentor and orient new staff members while maintaining knowledge in nursing practices.

Required Experience / Certifications / Licensure

• Current New York State LPN Licensure.
• BLS certification (AHA or RQI required).
• Minimum of 1 year experience in an LPN capacity, preferably in a Long-Term Care facility.
