Job Description

This position is within the RN Case Management unit. The unit comprises a mix of registered nurses (RNs) and social workers (SWs) dedicated to providing excellent care in a fast-paced environment.

Responsibilities

• Manage and coordinate patient care plans.
• Facilitate discharge planning and resource allocation.
• Collaborate with healthcare professionals to optimize patient outcomes.

Required Experience / Certifications / Licensure

• Experience in an acute care setting.
• Current RN license in Florida.
• Certifications: BLS and ACLS preferred.
