Part-Time LPN - Evenings

Assignment Duration

This is a 13-week contract position.

Schedule

Part-time evenings (2 or 3 shifts per week), with every other weekend. Holiday availability is required.

Job Description

The LPN will provide both direct and indirect skilled nursing care for assigned residents under the supervision of a Registered Nurse. The individual is expected to demonstrate knowledge of patient care appropriate to the age of the residents served, including adult and geriatric populations.

Responsibilities

• Collect ongoing data regarding residents' physical and psychosocial conditions and communicate findings to the RN and interdisciplinary team.
• Contribute to the development of individualized care plans that reflect nursing diagnoses and interventions.
• Evaluate resident outcomes and recommend revisions to care plans as necessary.
• Collaborate with residents, families, and healthcare team members to promote a safe and healthy environment.
• Maintain current knowledge and competency in nursing practice through education and in-service programs.
• Provide mentorship and orientation to new staff under the direction of the RN.

Required Experience / Certifications / Licensure

• Valid New York LPN Licensure
• BLS certification through AHA or RQI
• A minimum of 1 year of experience serving as an LPN in a Long-Term Care facility
