Job Description

We are seeking an experienced Registered Nurse Case Manager to join our team. This role involves providing acute care case management to patients within a hospital setting. The ideal candidate will be knowledgeable in Meditech, Midas, and Interqual.

Responsibilities

• Manage patient care effectively during hospital admission and discharge.
• Conduct assessments and develop care plans based on patient needs.
• Collaborate with multidisciplinary teams to ensure optimal patient outcomes.
• Utilize clinical knowledge and resources effectively for decision-making.
• Facilitate communication between patients, families, and healthcare professionals.

Required Experience / Certifications / Licensure

• Acute (hospital) Case Management experience required.
• Valid RN license in the state of practice.
• Competency in using Meditech, Midas, and Interqual.
