Assignment Duration

This is a 13-week contract position.

Schedule

Days: Sunday - Thursday
Hours: 10am - 7pm or 11am - 8pm (Schedule can be flexible based on patient's needs)

Job Description

The Hospice Nurse admits patients to service and assesses the patient's and family's physical, psychosocial, environmental, safety, and developmental needs. They develop an individualized plan of care based on their assessment and document their observations and interventions related to the patient's environment. This role coordinates care with a multidisciplinary team, participates in the hospice program's quality assessment performance improvement program, and conducts follow-up visits as needed.

Required Experience / Certifications / Licensure

One year of Hospice experience is required.
