Day Shift: 5x8-Hour (08:30 - 17:00)

Job Description

The Hospice Case Manager is responsible for providing specialized care and support to patients in a hospice setting. This position requires travel within Burlington, Camden, and Gloucester counties to ensure quality patient care and support for families dealing with terminal diagnoses.

Responsibilities

• Conduct intake assessments and interviews of patients and families.
• Develop and implement comprehensive care plans aligned with patient goals.
• Provide education and support to patients and families regarding palliative care and end-of-life options.
• Conduct regular evaluations of patient needs and adjust care plans accordingly.
• Manage medication administration and symptom management, utilizing pharmacologic and non-pharmacologic interventions.
• Ensure compliance with Medicare state regulations and hospice policies.
• Assist in bereavement planning and postmortem care as necessary.

Required Experience / Certifications / Licensure

• Minimum of 2 years of Home Care Hospice experience.
• Experience with Epic charting system.
• Current BLS certification.
• Active RN license in New Jersey or Compact state.
• Ability to work in a fast-paced team environment and float among designated facilities.
• Knowledge of pain management techniques and cultural competencies in hospice care.
