Registered Nurse - Hospice Care

Job Description

The Registered Nurse (RN) in this hospice care position is responsible for patient assessments, care planning, and case management within a home hospice environment. The RN will ensure quality patient care and coordinate interdisciplinary communication while adhering to hospice regulations and insurance reimbursement guidelines.

Responsibilities

• Perform patient assessments to determine appropriateness for hospice care and identify learning needs.
• Develop and implement plans of care for patients and families.
• Ensure appropriate utilization of services and maintain visit productivity standards.
• Conduct discharge assessments and manage admission and re-visit documentation.
• Provide patient education using age-appropriate teaching methods.
• Assume responsibility for case management, revising care plans as required.
• Maintain a safe environment and adhere to infection control measures.
• Document case management activities and evaluate performance in regular meetings.
• Participate in performance improvement programs, providing orientation and mentorship to new employees.

Required Experience / Certifications / Licensure

• Must have two (2) years of experience as an RN in a home hospice environment.
• Current New Jersey RN License.
• Valid New Jersey driver's license.
• BLS certification required.
• Experience using an EMR with excellent critical thinking and communication skills preferred.
