Case Manager - 4x10 Days Shift

Schedule

Day Shift: 4x10 hours (08:00 AM - 06:30 PM). Must work 2 weekend shifts per 4-week period.

Job Description

The RN Care Coordinator is responsible for determining the appropriateness of hospital admissions and advocating, coordinating, and facilitating the interdisciplinary plan of care. This role aims to expedite medically appropriate, effective, efficient, and timely utilization of resources for maximum patient outcomes.

Responsibilities

• Conduct assessments to determine the necessity of hospital admissions.
• Manage and coordinate the interdisciplinary plan of care.
• Ensure proper discharge planning and assessment.
• Arrange post-acute services including Home Health, Skilled Nursing, Hospice, and more.
• Coordinate medication pre-authorization and transportation needs.

Required Experience / Certifications / Licensure

• Minimum of 2 years of Case Management experience with a focus on critical care.
• Strong experience with EPIC charting required.
• Current California RN license required.
• Basic Life Support (BLS) certification required.
• BSN preferred.
