Ambulatory Registered Nurse (RN) - Case Manager
Job Description
The Ambulatory Registered Nurse (RN) provides comprehensive, patient-centered nursing care in an outpatient clinic setting, supporting individuals and families across the continuum of care. This role utilizes the nursing process to assess, plan, implement, and evaluate care; administer medications and treatments; perform clinical procedures; and provide patient education and care coordination in collaboration with providers and interdisciplinary teams.
Responsibilities
- Perform comprehensive patient assessments and develop individualized plans of care.
- Implement and evaluate nursing interventions and patient responses.
- Administer medications and treatments safely and accurately per provider orders and policy.
- Recognize changes in patient condition and initiate appropriate interventions or escalation of care.
- Provide patient and family education related to care, medications, and discharge needs.
- Document assessments, interventions, and outcomes accurately and timely in the electronic health record.
- Facilitate referrals, follow-up care, and continuity of services.
- Participate in quality and performance improvement activities.
- Maintain a clean and safe working environment and report any defective or missing equipment.
Required Experience / Certifications / Licensure
- Current full, unrestricted Registered Nursing license in Alaska or other U.S. state required.
- Graduate of an accredited school of nursing; Bachelor’s degree preferred.
- Basic Life Support certification required.
- Case Management experience is required.
- Case Management certification preferred.
Schedule
This position operates on a Monday to Friday day shift.
Assignment Duration
This is a 13-week contract position.
